Mount Carmel Health Partners is a Clinically Integrated Network (CIN) composed of over 3,000 primary care, specialty physicians and advanced practice providers dedicated to improving patient care and clinical outcomes in central Ohio.

If you are interested in joining Health Partners, please click on the Provider Credentialing Form. Here you will enter the applicant’s information, upload documents, pay the application fee and submit the application.

Just a reminder that when submitting provider information to Health Partners on the CAQH application you are certifying that all the information is current, true, correct, accurate and complete. This information is submitted to all payors to ensure timely and accurate claims processing and directory updates. Please review your CAQH information and update appropriately prior to submitting to Health Partners for initial credentialing or recredentialing. Your cooperation is greatly appreciated.

The application fee is $200.00 for Physicians and Advanced Practice Providers. Once the application materials and initial credentialing fee have been received, the application will be thoroughly reviewed for eligibility. Should your application not meet eligibility criteria, the fee will be returned. The fee is non-refundable once the credentialing process has started. You will be notified if additional information is required.
