Patient Registry

The Mount Carmel Health Partners Patient Registry is an electronic health care tool that collects and analyzes clinical data from physicians, hospitals, laboratories, pharmacies, and other health care providers in order to supply our members and Mount Carmel hospitals with information they can use to provide better and more comprehensive patient care. This information is also used in pay-for-quality discussions with payors to determine the quality of care our network provides.

The patient registry utilizes clinical data from the claims acquired from members’ practice management system or billing companies, outside laboratories, prescription histories, and data from Mount Carmel Health System (and does NOT collect financial information such as charge amounts or reimbursement amounts). The registry also features an automated Physician Quality Reporting System (PQRS) physician reporting which can be submitted directly to the Centers for Medicare & Medicaid Services (CMS) for physician incentive payments.

The Health Partners Patient Registry can serve as a population management and point-of-care tool as well, providing users with the ability to access an intuitive interactive screen during a patient’s visit. It offers proactive intervention management features and provides comprehensive compliance data.

All physicians who participate in our Clinically Integrated Network agree to submit data and use the Health Partners Patient Registry, which is provided be McKesson (formerly Medventive). Each physician or practice determines to what extent they use the registry and how its features can best be utilized.
