2016 Annual Report

It is with great pleasure that all of us at Mount Carmel Health Partners present you with our annual report for 2016. We’ve been fortunate to enjoy extraordinary growth and success over the last year, particularly in the areas of clinical integration, accountable care, and quality improvement, and are pleased to share some of the highlights with you.

We spent the last several years preparing our organization for the challenges of the ever-changing landscape of healthcare reform, and all the evidence suggests that our strategies of preparedness and foresight were on target. In the past year, we exceeded our goals for provider engagement in our clinically integrated network. We expanded our accountable care partnerships to nine, and our population health management efforts were greatly enhanced by increasing both our programming and staff.

As a result of this remarkable growth, Health Partners was able to make significant investments in the future in the areas of infrastructure and data analytics. We also increased our commitment to patients by supporting our member physicians in delivering the highest-quality healthcare as effectively and efficiently as possible.

We owe a great deal of our success to the ongoing support of our member providers — primary care and specialty physicians, advanced practice providers and hospital partners. We couldn’t do what we do without each one of them. Our future is bright as we continue to collaborate across the continuum of care to provide our patients with the highest-quality healthcare.
